$5,000 Scholarship Contest Step 1: Read Contest Information Read complete contest information before applying. Please read all requirements carefully. Entries that do not meet ALL requirements will be automatically disqualified from the contest without notification to the student. Applicant Eligibility Requirements · Open to citizens and aliens legally residing in the USA with High School Diploma, GED or equivalent. · Scholarship payable to the winner’s college/university or certified trade school in the United States; winner must enroll or already be enrolled for Spring 2015 session. · Winners not based on financial need · Relatives of AFSA staff or board members may not enter. Requirements o Read the "Fire Sprinkler Essay" (pdf) or (html) about automatic fire sprinklers. o Complete registration page with all requested information. Before scholarship funds will be awarded, all information submitted on the application by the entrant will be verified. If ANY false or misleading information is submitted on the application, the student will be disqualified without notification to the entrant. · Take a ten-question multiple-choice test. This is an open-book test and you may print the essay beforehand for reference during testing. For each question answered correctly, entrants will receive one (1) entry into a drawing for one of five $1,000 scholarships. For each question answered incorrectly, you will be given one more chance at the end of the exam to answer those questions correctly. A total of ten (10) entries into the drawing is possible if the entrant answers all ten questions correctly. · Each entrant is allowed only one opportunity to take the online test. Any attempt to enter again under a different name, address, etc., will automatically disqualify the entrant without notification to the entrant. · Entrants selected as winners are required to submit the name and contact information of their chosen college/university/certified trade school* for verification of registration and payment of the scholarship to the school. Information must be submitted no later than December 12, 2014 or the scholarship will be considered unclaimed and another winner will be selected. *A 2- or 4-year college/university/technical/career/online school based in the United States that has completed & received accreditation from an agency recognized by the US Department of Education & is on the list found at http://ope.ed.gov/accreditation or http://www.chea.org Deadline Entries will be accepted from 12:00 p.m. (noon) CDT April 9, 2014 until 12:00 p.m. (noon) CDT August 27, 2014. Prizes A total of $5,000 is awarded. Five (5) winners will receive a scholarship of $1,000 each. Only one $1,000 scholarship will be awarded to any single individual from this contest. Odds of winning are based on total number of entries received. Past winners of AFSA's High School Scholarship Contest or AFSA's Second Chance Scholarship Contest cannot win this contest. The scholarship is paid directly to the educational institution in the entrant’s account to be used for tuition and/or books for the Spring 2015 session. Winner Notification Due to the number of entries received, AFSA will only notify winners via email and/or phone. AFSA will contact each winner by email and phone within fourteen (14) business days of contest end date, using contact information given by the entrant during application process. Winners have ten (10) days from date email is sent/phone message is left to respond and verify they meet the eligibility requirements as defined on the scholarship contest Web site. "Second Chance" winners will need to fax, scan, or mail a copy of their U.S. birth certificate, U.S.-issued passport, or green card WITH a copy of their citizenship proof from country of origin. (Two forms of ID required.) Once you are confirmed as winner, AFSA will send you the appropriate college/university/trade school forms to complete for a check to be issued to your chosen school and deposited into your student account. No funds will be issued directly to individuals. All forms and processes must be completed within sixty (60) days of contest end date or your scholarship is forfeited. Winners’ names will be posted on this web site on October 1, 2014. Application Process Applications will only be accepted online through the AFSA scholarship web site (www.afsascholarship.org). Entrant must provide all information requested to be able to submit his/her entry. Step 1: Read all contest information. Step 2: Read the "Fire Sprinkler Essay" You may print the essay for reference if you choose. Step 3: Click on “Take Test Now” button and fill out the entrant information section. You will need to provide the following information: - Full Name - Home Street Address - City, State, Zip Code - Home Phone (with area code) - 2nd Phone Contact (optional) - Email Address - Birthdate - High School Graduation Date / or GED Certificate Award Date (Month/Year) Taking the Test: Select answers for the ten-question multiple-choice test. This is an "open book" test. You may refer to a printed copy if you choose, or use the essay text online to find the correct answer to the question. Click on "submit" button. You will immediately receive notification of questions answered incorrectly. For each question answered incorrectly, you will be given one more chance to answer those questions correctly. This is your LAST CHANCE to refer back to the information in the essay and select the correct answer to the question/questions that you missed. Click on the “Submit” button. You will immediately receive online notification that your entry has been submitted. The number of questions answered correctly equals the number of times your name will be entered into the scholarship drawing. You will NOT receive an email confirmation. Step 2: Read the Essay Print the essay for future reference or read the essay online and refer to it while taking the test. Printing out the essay is highly recommended.   1. Darn I wish I knew about this when I went to college last fall. I could have used the $5000 for my tuition, on well, I suppose I can share this to some of my friends who may be attending college very soon. Thanks for sharing.
